Halton Healthcare has launched a new virtual care pathway through SeamlessMD, a secure online platform that connects palliative care patients with their care team and supports them at home.
Led by the Community Integration & Primary Health Care Program in collaboration with the Palliative Care Team, the initiative provides education, symptom tracking, and reminders to help patients and caregivers manage comfort and wellbeing.
“This digital tool allows us to provide more consistent, compassionate, and proactive care while easing the journey for both patients and families,” says Dr. Tarek Kazem, Palliative Medicine Division Lead.
Since its launch, 92% of patients have activated their accounts, 83% regularly complete symptom check-ins, and all respondents said they would recommend the program.
Patients receive personalized education, wellbeing check-ins, and resources to help manage pain, fatigue, and emotional challenges.
“This partnership allows us to extend critical supports beyond the hospital walls, helping people remain safe and supported in their homes,” adds Elma Hrapovich, Program Director, Community Integration & Primary Health Care.
Halton Healthcare plans to expand enrollment, refine content, and assess outcomes over the coming months.
